Mix sugar and cornstarch together in a pan.
Add the 2 cups of water over moderate heat, stirring until slightly thickened. (May be cooked in microwave.) Combine egg yolks with juice and beat.
Stir a small amount of hot cornstarch mixture into yolks.
Stir all of yolks into cornstarch mixture.
Cook until thick, stirring frequently.
Add lemon extract, butter and vinegar.
Shell and let cool.
Cover with meringue and brown.
